Fort Sanders Regional Medical Center Quality Initiatives
- Institute for Healthcare Improvement "100,000 Lives" Campaign - All Covenant Health hospitals are among nearly 2,000 hospitals nationwide which are members of an IHI campaign to implement improvements in care including deployment of rapid response teams, delivery of evidence-based care for heart attack, preventing adverse drug events, preventing central line infections, preventing surgical site infections and preventing ventilator-assisted pneumonia.
- Transformation of the ICU Project - Regional and other Covenant hospitals are partnering with other critical care units nationwide to provide the best quality and safest care to critically ill patients and their families. Participants include community hospitals and tertiary teaching centers such as Johns Hopkins and Baylor Health Care System.
- Stroke Centers of Excellence - Covenant Health hospitals are working diligently to provide excellent care for stroke patients through the use of evidence-based treatment protocols, JCAHO standards, and other internal quality measures.
- Women's Heart Care Initiative, a Covenant Health effort to raise awareness of heart disease in women and encourage early diagnosis and treatment
- Covenant Health Balanced Scorecard of quality indicators
- JCAHO National Patient Safety Goals
- Leapfrog Patient Safety Guidelines
- Institute of Healthcare Improvement (IHI) Wound Care Initiative - Fort Sanders developed a wound care management program for our inpatients in 2005 to reduce the number of acquired wounds and to better identify wounds present upon admission. Due to our activities, we were able to drop wound rates to 1%.
- Medication Reconciliation - in response to new JCAHO standards, a medication reconciliation team was developed. Comprised of representatives from pharmacy, nursing, information technology, quality improvement, risk management, and management engineering, the team has implemented several programs to improve medication reconciliation for our patients.
- VHA Collaborative - Fort Sanders Critical Care units have been participating with the VHA collaborative Transformation of the ICU since August 2002. Both blood stream infections and ventilator associated pneumonias were decreased by 50%. A rapid response team was established this year designed to decrease code blue situations by early rescue.
